SummaryThis position will serve as an academic primary care provider (PCP) in a Patient Aligned Care Team (PACT) with a teamlet comprised of a provider (MD/DO) - RN - LVN - and a Clerk. The team will provide primary and acute care services in an outpatient setting to include chronic disease management and preventive health care.
