WHO WE ARE: Establishedin 2001, South County Community Health Center, Inc., dba: Ravenswood Family Health Network (RFHN) is a Federally Qualified Health Center with 5 clinical locations, serving 4 communities; East Palo Alto, Palo Alto, Mountain View and Sunnyvale. Our diverse patient population is Latino, African American, and Pacific Islander with some sprinkling of Asians and Southeast Asians. We provide integrated full scope primary and preventative medical, dental, behavioral health and social services to over 24,000 patients a year. We also offer optometry, chiropractic, podiatry, imaging, lactation consultation, and pharmacy services.
We partner with Stanford Healthcare, Lucile Packard Children’s Hospital, San Mateo Medical Center, El Camino Hospital and Valley Medical Center to connect patients to specialty care.
WHERE WE ARE LOCATED: RFHN clinical sites are located in Silicon Valley at the southern end of San Francisco Bay which is southeast San Mateo County and northern Santa Clara County. We have some of the best weather in the Bay Area, mild winters and warm summers. The wetlands, hills and mountains surrounding the Bay Area provide many opportunities to be in nature. Restaurants with diverse ethnic gastronomical delights are in abundance.
