Open Door Community Health Centers’ (ODCHC) Behavioral Health Department is committed to evidence based and timely mental health treatment services for established patients. Medical providers, care teams, and support staff utilize Behavioral Health Clinicians (BHC) for collaboration on patient care, and for consultation on difficult mental health issues. Open Door’s integrated behavioral healthcare model of practice is patient-centered, sessions focus on improvement of overall health outcomes, while addressing issues of depression anxiety, and addictions. Treatment is normally short-term unless clinically justified for more in-depth sessions.
Open Door Community Health Centers (ODCHC), a federally qualified health center established in 1971, is a network of 12 clinics in Humboldt and Del Norte Counties on California’s north coast. ODCHC is the medical home for more than half of the area’s 160,000 residents. We employ more than 100 licensed medical, dental and behavioral health practitioners. Our behavioral health clinicians (BHC) work in the medical care teams which include physicians, PA & NPs, RNs, MAs, Care Coordinators, Nutritionist, and more. We strive to make sure everyone can work at the top of their scope. ODCHC is California’s largest provider of primary care services north of San Francisco. ODCHC uses the EPIC electronic health record system (since 2008) and has teleconference and telehealth capacity in all sites.
Our BHCs are an integral part of the primary health care teams assisting in identifying and managing patients with behavioral health needs. Our BHCs provide short term care as well as warm hand-offs, assist with school based care, as well as the many other specialty programs provided. Our care teams see a full-spectrum of chronic and acute care needs including comprehensive family medicine, internal medicine, women’s health, men’s health and reproductive health; pediatrics and teen health care; chronic illness management; geriatric care; buprenorphine treatment for opioid dependence (and chronic pain); integrated behavioral health services; referral and follow-up for specialty care locally, regionally or at urban medical centers (UCSF, UC Davis, Stanford, USC) and telemedicine. Additionally, dental services; HIV treatment and management; HepC treatment and management; pre-natal and OB care, transgender services; adult and child psychiatry; and outreach and care for homeless services are provided. Referral and follow-up for necessary specialty assessment, diagnosis, consult and treatment available through local, regional and urban medical centers (UC San Francisco, UC Davis, Stanford, USC) and telemedicine. Clinic provides geriatric care, diabetes education and management; hypertension management; chronic pain management (with and without opioids), migrant health, homeless health, and teen health services. Additionally ODCHC has a rural track Family Medicine Residency Program and PA/NP Residency Program which a dedicated BHC currently supports. We have several newly built facilities, and the next already on the drawing boards.
