Assists CEO and provides administrative support to the Avenal Community Health Center (ACHC) Case Managers. Able to manage multidisciplinary care teams in the effective care and case management of patients.
High School diploma required.
Preference for Bachelors degree in Social Work, Psychology, or in relevant field or 2-3 years of community outreach experience.
2-3 years experience in working with individuals with chronic health conditions, and/or behavioral health conditions and/or Substance Abuse Disorder and/or homelessness. To help them access needed supports and resources.
Bilingual in English/Spanish, preferred.

Performs other related duties as assigned.
Requires a full range of body motion including handling and lifting, manual finger dexterity and eye-hand coordination. Involves standing, sitting, and walking. Requires normal visual acuity and hearing.
