POSITION TITLE:Program Coordinator for Health Care Coverage & Access

GENERAL STATEMENT OF DUTIES:Under the supervision of the Director of Health Care Coverage & Access, the Program Coordinator is responsible for providing a wide range of program, administrative, and clerical support activities in the areas of enrollment in health care coverage and insurance, outreach and education, care coordination, and clinic operations for SFCCC. The Coordinator must support SFCCC’s mission to ensure that people of all income levels have access to quality, comprehensive, community-based primary care that is culturally and linguistically appropriate and focused on patient needs. SFCCC and our 12 partner health centers care for more than 10% of San Francisco’s population, serving low-income, homeless, immigrant, racially and ethnically diverse, LGBTQ, HIV-positive, and senior San Franciscans.
REPORTS TO: Director, Health Care Coverage & Access
DUTIES AND RESPONSIBILITIES:
Serves as point of contact for SFCCC peer groups of Outreach & Education staff, Care Coordinators, Clinic Managers or other groups as necessary and distributes information to participating clinic staff.
Provides administrative support for Policy and External Affairs department, including but not limited to:organizing meetings and events hosted by Department and producingminutes.
Collects and organizes Navigator Program data, including tracking SFCCC and sub-recipient grant deliverables and data, uploading and verifying data submissions into a SalesForce or other platform to be submitted monthly and quarterly for Navigator grant from CA Department of Health Care Services.
Maintains and updates electronic filing systems.
Conducts some program-related research (e.g. search foroutreach and education materials and/or care coordination materials to be used for Medi-Cal, Healthy San Francisco, etc.).
Compiles and assists in the collection, data entry, and presentation of enrollmentand other data and educational materials.
Assist partner clinics with someoutreach and enrollment activities in their communities as appropriate.
Complies with SFCCC Corporate Compliance and Personnel policies and procedures, including HIPAA, and reports any conduct or event that potentially violates legal, compliance or personnel requirements. Fully participates in any related investigation or proceeding.
Performs other duties, including clerical and administrative support, as assigned.
WORKING CONDITIONS:Monday – Friday, 9am – 5pm. Occasional evening or weekends may be needed.
Standard office setting with extended periods at a workstation and periodic use of office equipment.
Occasional local travel to partner clinics and other locations for meetings, to provide technical assistanceand/or assist with outreach and enrollment events, potentially including enrolling eligible applicants into health care coverage and insurance.

REQUIRED EDUCATION AND SKILLS:
At least two years of college education.
Strong interpersonal and communication skills; English literacy and demonstrated ability to effectively communicate verbally and in writing.
Demonstrated ability to work effectively, cooperatively and collaboratively as a member of a team in widely different environments, among diverse organizations and with diverse staff composition.
Demonstrated data entry skills and attention to details.
Basic understanding of community health.
Proven proficiency in MS Office programs and some experience with programs that store, combine, manage, validate, analyze, and produce graphic representations of large data sets.
Competent with social media use and posting, web site navigation and blog posting processes.
Flexibility to adapt to changing priorities and assignments.
Problem-solving skills and ability to work independently and under pressure to meet deadlines.
Demonstrated ability to effectively manage time, interpersonal relationships, resources and information and to exercise sound independent judgment.
REQUIRED EXPERIENCE:2 years of work experience.
ADDITIONAL DESIRED EDUCATION, SKILLS AND EXPERIENCE:
Bachelor’s degree is desirable with preference for study in a field related to this position.
Work or volunteer experience in a health care related organization or a community-based organization.
Experience living, working,or volunteering with diverse, low-income, communities of color.

San Francisco Community Clinic Consortium's 12 member clinics serve over 112,000 low-income, uninsured, and medically underserved people per year. The clinics are located strategically throughout San Francisco to meet the health needs of the people and communities they serve. Comprehensive primary, preventive, and ambulatory care services are provided by a team of doctors, mid-level clinicians, nurses, dentists and health practitioners under the supervision of a medical director.
