We have an exciting opportunity to bring onboard a Community Integration Services Director.  The Community Integration Services Department seeks to integrate the treatment of health and social issues as a vital pathway toward realizing the mission and goals for EVCHC. The purpose of this position is to execute procedures and services that improve overall health and well-being for vulnerable populations with high care needs and experiencing various forms of insecurity.  This is done through the coordination of healthcare and social services, addressing social determinates of health and facilitating access to and navigation of relevant health care systems.

Essential Position Functions and Duties: 

• Provides leadership and planning to ensure appropriate collaborations and contracts are in place to serve homeless individuals. 
• Oversees homeless projects operating within EVCHC.
• Oversees contract compliance and compliance with EVCHC mission, vision and policies and procedures as appropriate.
• Works closely with Homeless Case Manager and local networks to locate support services and resources for high barrier individuals and families within the San Gabriel Valley and Pomona.
• Establishes internal and external processes for serving homeless patients and creates an environment of accountability.
• Implement toolset and processes to address, monitor and track Social Determinates of Health for all patients.
• Provides support and communication with internal teams to ensure appropriate and comprehensive delivery of services to homeless patients. 
• Collaborates, as required in the preparation, submission of grant applications related to services for high barrier individuals and ensures program compliance and administration once the grant is received. 
• Maintains current working knowledge of services for homeless environment, as it relates to healthcare and best practices. 
• Leads and directs others through change processes.
• Champions new ideas.
• Performs other job related duties, as may be assigned.

Skills and Qualification Requirements: 

• Master’s Degree in related field. Bachelor’s Degree will be considered with a minimum of five years management experience in related field.
• At least 5 years of experience working with homeless population. At least 3 years in a management capacity. 
• Strong problem solving and analytical skills; systems thinking.
• Excellent communication skills (oral and written) and interpersonal skills for frequent contact with internal customers and external stakeholders.
• Ability to positively influence teams.
• Ability to multi-task and prioritize daily workload
