We have an exciting opportunity to bring onboard a Community Integration Services Director. The Community Integration Services Department seeks to integrate the treatment of health and social issues as a vital pathway toward realizing the mission and goals for EVCHC. The purpose of this position is to execute procedures and services that improve overall health and well-being for vulnerable populations with high care needs and experiencing various forms of insecurity. This is done through the coordination of healthcare and social services, addressing social determinates of health and facilitating access to and navigation of relevant health care systems.
